I was today years old when I learned about Spirit Island

Betrayal at Baldur's Gate applies the Betrayal at House on the Hill system to a D&D city setting. The omen cards, haunt trigger, and two-phase structure carry over with thematic reskinning. The new haunt scenarios are specifically balanced for the D&D setting and are on average somewhat better balanced than the base game.

For groups that love the Betrayal system and D&D: this is the version to own if you have to choose one. For groups that love the horror house theme specifically: House on the Hill maintains its advantage in atmosphere. For groups that own both: the scenario variety across both boxes creates a combined experience that neither offers alone.

The Widow's Walk expansion for House on the Hill and the system's third floor expansion have created a combined Betrayal experience that is larger than any single box.
